You can get it to turn on automatically, when you open your media player... mine does.

I'm just looking over it and there doesn't seem to be any option to do it from the program. I'm thinking that when I first installed it it asked me if I wanted it to come up on startup of my media player.
As well, I use Windows Media Player and have Last.fm Plugin checked on in the tools->plugin menu.
