i didn't get to really get what you mean despite the fact that i've been staring at the picutre for over 30 seconds now..hehe..but that's good news. at least it's not real cause if it were, it's kind a scary. God giving us the middle finger sign.

Obviously, it is computer edited.

But there are real cases. I took this one myself about two years ago. And it's totally natural. No retouch at all:
