Re (b), I watch all modern films with subtitles on. There's way too much mumbling etc. in films.

I'm the same exact way. If I read the lyrics along to the song, I'm usually fine. but if I've heard the song quite a few times without reading the lyrics, I either hear nothing intelligible or I hear things they are definitely NOT saying.
