With designs and patterns its a far easier thing to fumble through without getting it perfect, because the dimensions don't need to be exact. With a face, if its drawn even slightly wrong it will look nothing like the thing it is supposed to because theres a very, very clear model people can compare to.

Also, tattoos with a lot of details (like, say, a photo-realistic portrait) fade and blur badly over time. In twenty years, even a perfect tattoo of Kurt Cobain is going to look like a blob of ink in the shape of some unknown person.
