That, my friend, would be an artisanal grilled cheese from Tom & Chee, a deli that sells nothing but tomato soup and grilled cheese. Everything they make is freaking delicious.
My particular sandwich was a mushroom-swiss grilled cheese on rye bread with red peppers and sundried tomatoes. I believe the sandwich in the picture is their grilled mac-n-cheese.
