It's this. Some meds can be taken with other meds that counter side-effects too. But yeah, basically the problem is that depression is an umbrella term for a lot of different pathological states and we don't understand them all well and people already have variations in physiology that makes finding a drug program harder. As you noted, drugs have to be tried on a case by case basis by the psychiatrist as they closely monitor the patient, until something (hopefully) works. Some people never find a drug program that works for them. But for others, it can be a quality-of-life saver.
