They're not supposed to.

There are usually two kinds of rounds used on a movie set, blanks and dummy rounds.

Blanks are rounds that have no projectile (the bullet) but still have gunpowder to produce the bang but nothing propelled forward.

Dummies are rounds that have the projectile but have had either the primer struck, or no gunpowder, usually both. These are rounds made to "look" real say in a chamber of a revolver during a close up of the gun.

Mistakes are made when somebody mixes up these methods. That's what happened with Brandon Lee. They accidentally left a bullet in the chamber and inserted blanks and when they fired the blank, the bullet that was lodged in the barrel shot out and killed Lee.

They don't know for sure what happened with Baldwin yet, but it's mostly something careless like described above.

Or, they brought real live rounds on set. That isn't impossible either.
