allright im going to solve it
he was on an island or something in the ocean, which would explain him swimming in his clothes..but he was starnded there with his friend, named seagull
a long time passes with out food, so a fight ensues, the man kills seagull, and eats him naming this food seagull soup, then after awhile when his food runs out, he swims back to a beach somewhere .. he finds the restaurant..and he is so starving he seas seagull soup and orders it.. but then he realizes that he ate his friend and he called it seagull soup, so he got so sick inside, that he couldnt bear it.and he ran out. and got hit by a car or something

We went back there and they had come and hacked off every inoculated arm. There they were in a pile. A pile of little arms. And I remember... I... I... I cried. I wept like some grandmother. I wanted to tear my teeth out. I didn't know what I wanted to do. And I want to remember it. I never want to forget it. I never want to forget. And then I realized... like I was shot... like I was shot with a diamond... a diamond bullet right through my forehead. And I thought: My God... the genius of that.
