yeahh i agree with thebassist
go on a piano, find where your voice "cracks" (meaning where you switch from your chest to your head voice) and keep practicing those notes until the notes blend together.
to you, the chest voice will always sound a lot better than your head voice, but just keep practicing, and soon they should sound good together.
practice, practice, practice..
