According to the BBC, that's the total number of drug addicts in Russia. It's not just heroin addicts, though they are a large part of the picture. Also, you have to remember that Russia is a huge country. 2.5 million is only 1.8% of their population. I don't have the statistics for rates of drug addiction in other countries but it doesn't seem like 1.8% would be an abnormally high figure for anywhere.
