Default Student Evaluation

I want to give me students an evaluation. That way they can go over it with their parents. Any suggestions?

From what I understand, that's not necessary until over the age of 40. Maybe he can instruct his students in the proper method of evaluating their parents and to be fair and equal, he should probably also teach them the supple art of using a scapula.
