I'd say it's probably the most depressing show on TV actually. I don't think I've ever seen an episode where the person recovers, invariably they agree to treatment and then a little note pops up that the end of the show informing you that the person went right back to using a few months later.

I've seen quite a few episodes where the subject ends up staying clean. It's a great show because it's a TRUE reality show. It's pretty raw, and some episodes can be hard to watch. But, ultimately, it's rewarding. I feel like I've learned a lot about addiction and dependency just from watching that show.
